The 2013 Disease-Specific Care Certification Manual features all the updates and provides everything your disease-specific care program needs to attain or retain core or advanced certification. Core Joint Commission certification helps you build a structure to deliver consistent care with better outcomes, while advanced certification incorporates optional clinical requirements developed in collaboration with industry experts. It includes: Core standards for all disease-specific care programs delivering clinical care directly to participants; details about The Joint Commission"s certification process, performance improvement, sentinel events, Certification Participation Requirements, and more; standards for program management, delivering clinical care, supporting self-management, clinical information management, and performance measurement; keys to successfully using the manual for survey preparedness; eight advanced certification programs, allowing you to focus on the most applicable requirements; summary of changes made since the previous edition; and icons to help you navigate scoring, criticality, and documentation requirements. Current Core Certification Programs: These are disease-management programs that provide clinical care and services directly to participants in hospitals, long term care settings, home care organizations, health plans, integrated delivery systems, rehabilitation centers, physician groups, and disease-management service companies.
